After 1 month, looks like training and going cautious on away games did some improvement. One more question, do you ever go attacking or very attacking on home games when you are clear favorite?

OMG, training never crossed my mind. It makes a lot of sense. In my Dinamo Zagreb save, I was using Hood Gaming schedule, although it is bit older video. Hood Gaming schedule
In my Leeds save I brought couple of really good coaches so I delegated training to them, since no one complained and attributes were going up on majority of players I left it at that.
By started from scratch you mean I didn't continue an older save? If that's the question then yes.

Tested it with Dinamo Zagreb with the default database, and it was amazing, in CL quarterfinals lost to Man City on goal difference.
But then tried it with Leeds with the new player database, at it's going hot-cold. Losing to Brentford 5-1, Chelsea 7-3. My entire defense was below average in 70% of the games. Meslier is non-existent, I know that is not the tactic's fault and I know it's not plug-and-play type of tactic. Started second season with 4 pts in 3 games and I was the favorite in 2/3
I tried few tweaks, add more players on defend and lowering lines, but it made no difference Just wondering what tweaks could help.
Since it did wonders with Dinamo where I had worse players and did 0 changes to tactic.
