It's an known issue that some PIs and TIs visually bugged in FM21 so if you notice something weird then probably, it's a visual bug.

I'd say FM Scout Editor is a quite scouting oriented editor. It can't do some editing things that the official FM in-game editor or FMRTE - https://www.fmrte.com/ can do.

You don't need any luck only if you are Barcelona and you play vs an amateur club but even in that case there's still like 0.0000001% probability of losing.
If your team aren't much better than your opponents then even with the most effective tactic there's still a huge room for the luck to kick in.

I've tried v4.0 with Dortmund and it worked great.
Just for the record, healmuth's AP&DM tweak is literally Phoenix v1.0 tactic with 2 tweaks:
1) DPL(S) was changed to DM(S)
2) CM(A) was changed to AP(A)
If you changed AP(A) to BBM(S) and DM(S) to RGA(S) then there's nothing left from healmuth's APDM tweak
Probably, that's why people asking you why you named your tactic APDM tweaks if there's nothing left from that tweak
It makes sense to name your tactic Phoenix v1.0 BBM(S) & RGA(S) tweak or something like that.
